All appointments must be made with a valid referral from a medical practitioner. General Practitioner referrals are valid for a period of 12 months after which time a new referral will be required if ongoing appointment needed. Referrals by a specialist such as your obstetrician are valid for 3 months only.

Things to bring to your appointment:

  • Child Health Record book (“green book”)
  • Reports or letters, e.g. school, medical, psychology, speech therapy, audiology, Xrays
  • List of medications (dose and strength)
